Perioperative and Rehabilitation Older Adult Care Pty Ltd is a Perth practice for older adults whose surgery is only half the story.
The gap is not in the theatre — it is on either side of it
Surgical and anaesthetic care in Western Australia is excellent, and an older adult having an operation today is in good hands on the day. What is thinner on the ground is the work that surrounds it: the conditioning that should happen while a person is on a waiting list, the geriatric assessment that should reach the anaesthetist before the morning of admission, and the rehabilitation that should keep going after the hospital has discharged its responsibility.
That is the space this practice occupies. We are not a substitute for a surgeon, a hospital or a general practitioner. We are the part that holds the plan together across all of them, and keeps it pointed at the same outcome: the person going home, and staying there.
We are based in Perth’s western suburbs, at postcode 6016, and work with people across the Perth metropolitan area.

Four commitments we hold to on every episode of care
They are unremarkable individually. Held together, across the whole pathway, they are what changes how a recovery goes.

Plain language, every time
Older adults and their families get told what is happening in words they can repeat to someone else. If a plan cannot be explained in a phone call, it is not finished.

Function is the measure
We assess and re-assess what a person can actually do — walk, stand from a chair, shower, manage their own tablets — and set goals against those, not against a session count.

One plan, shared with everyone treating
Assessments and updates go to the surgeon, the anaesthetist, the general practitioner and the ward — with consent — so nobody is working from a different version of the person.

The follow-up is part of the job
Discharge is the point at which most plans quietly stop. Ours carries on into the first weeks at home, because that is when the ground is most likely to be lost.
